About the Provider Pathway

One:carepath has been developed for adult patients (≥18 years of age) with the following advanced-stage (non-curative) conditions:

Chronic Obstructive Pulmonary Disease (COPD) Heart Failure
(HF)
Liver Cirrhosis Advanced Chronic Kidney Disease
(Advanced CKD)
Stage 3 or 4 Cancer

These patients are at high risk for deterioration and increased mortality. The goals of treatment are to prolong active, independent life by reducing symptom burden and delaying the progression of the underlying condition(s).

For most patients, appropriate chronic disease management and support is within the primary care setting.

If your patient has a confirmed or suspected diagnosis of one or more of these conditions use this pathway for evidence-based decision making.
