Heart Failure Management

The AHS / Specialist Link, Calgary Zone Enhanced Primary Care Clinical Heart Failure Pathway is a best-practice clinical pathway for management of heart failure relevant to the primary care medical home that includes a flow diagram and expanded details. It will be updated to reflect new evidence (see Canadian Heart Failure Society- Guidelines and Clinical Updates).
